4. Select: Remove module(s) to exit from NHVAS

5. Select: the module(s) you want to exit from

6. Add: additional comments regarding your exit application. For example, if you have requested an extension for any module you will be retaining

7. Complete the declarations and click: Submit Application

8. Click: Done to return to the home menu
